  5. Marksheet of your 12th grade as well as the copy of degree from a UG examination.
  6. MCI/NMC/State Council registration certificate, proof of internship completion.

  8. (Driver’s license, PAN card, voter ID, government or PSU card, Aadhaar card, passport) Photo ID.
  9. 6 passport-sized photos. The face must match the photo on the registration form.
